Karlshagen is not one of the oldest municipalities on the island of Usedom. People who specialized in herring fishery were the first to settle at this place in 1829. It lies on a narrow strip of land like several other seaside resorts on Usedom. The yacht harbour on the river Peene is therefore only 2 kilometres away from the large and sandy beach on the Baltic Sea.

The former fishing village has been a seaside resort since 1885 - except for a period of 9 years: Between 1936 and 1945 no one could spend his holidays at Karlshagen, because the whole northern part of the island had been declared a restricted zone of the rocket research centre at Peenemuende. A memorial for the dead of air attacks in 1943 and 1944, which can be found near the entrance of Karlshagen, today strongly recalls those hard times.

During these attacks the seaside resort and its pre-war architecture were almost entirely destroyed. Today Karlshagen gives the impression of a modern and well-kept place. The townscape is characterized mostly by two and three storey tasteful houses, but there are also some tenement blocks dating from the times of the communist German Democratic Republic. These large buildings have all  been renovated and improved in appeareance.
Karlshagen completely depends on the flourishing tourism sector, for there is not any other noteworthy branch of industry at this place and its surroundings.
